Honestly, the best tube integrated meeting and exceeding your criteria is the Audio Note Cobra. We have one in our system that beautifully runs our Quad ESL-57s. Best sounding that we have found for our very picky speakers.

I don't know why particular design criteria is important, such as single-ended, class A, or even balanced--it is the sound that counts.  The Cobra is okay to me, but I like the comparably priced and lower cost offerings of Synthesis Audio more (I heard direct comparison of the Cobra and the slightly cheaper Synthesis alternative that had a quite good built in DAC too.  If I were interested in Audio Note, it would be the Oto or Soro, but neither has remote control of volume.

@larryi certain design criteria are important to me because I find Class A sweeter than A/B & Balanced because it doesn't make sense to have a superior DAC & not make full use of it...
However it looks like I'm going to have to compromise on the class of operation as Class A tube amps seem to be pretty much unavailable in a balanced configuration where as I have a few decent choices in Class A/B PP with all the other features I want so it is likely I will end up going that direction...

Balanced Audio Technologies makes fully balanced integrated tube amps.  I have not heard their latest offerings so I don't know how they sound.  I am not a big fan of the 6C33C-B tube, but one never knows how a particular amp will sound.
