Syn Res Ground Cable quality problems

I recently purchased a Synergistic Research Active Ground Block SX and have had two ground cable failures within two weeks. First the RJ 45 connector housing got stuck in my PhoenixNET then the mini banana came off the pin on another.  I’ve been concerned this might happen as several of the cable connections have lateral play and seem to be connected by just a crimp.  Combined with how tight the pins are grabbed by the Ground block I’m concerned that removing any more will result in the same problem.  Any similar problems?
