As someone already mentioned, balanced inputs require double the circuitry, but the attached link provides just about everything else you want.

I have Rogers High Fidelity 65V-1 integrated amp that I am super thrilled with to drive my Klispch Cornwalls.  It is SET, pure Class A and is also a headphone amp BUT, it does not have BALANCED inputs.  Most importantly, it is self biasing and can take EL34 and KT88 out of the box and can run either Ultralinear or Triode mode. It uses an iPAD app to control (input select, volume, Triode vs Ultralinear etc)  as there is physical remote available.  Not ideal but good enough for me.  Also, you must have a load on the speaker terminals at all time when running the amp (including using it as a headphone amp)

Roger Gibboni's products are built like a tank, in the US of A (either Massachusetts or NY) .  He doesn't do much advertising / promotions and is always under the radar but his prices are "reasonable" by high end audio standards..  I believe he is, for all practical purposes, a one man show.

GOOD LUCK !

+1 for a Rogers High Fidelity Integrated amp. I have a bigger model in his line up ( EHF 200 Mark II) & really enjoy it. It’s built to a very high standard, sounds excellent & been trouble free for several years now.

VAC Sigma 170i preowned would perhaps fall in your price range. Available with balanced inputs, remote, auto bias but alas, no headphone.

FWIW – not SE, but the Linear Tube Audio z40+ ticks all of the other boxes, and is the best amp I have employed over nearly 50 years in the game.
